We’re seeking a dedicated and detail-oriented MDS Coordinator to join our growing team! This role is essential in ensuring accurate assessments, compliance with state and federal regulations, and optimal care planning for our residents. Success in this position requires reliability, strong organizational skills, and a collaborative approach to interdisciplinary coordination.
Your Impact as an MDS Coordinator
In this role, you will:
- Coordinate Resident Assessments: Schedule and oversee admission, annual, quarterly, Medicare, and change-of-condition assessments, ensuring timely communication with nursing and other departments.
- Ensure Regulatory Compliance: Maintain adherence to Federal and State guidelines for comprehensive assessments (MDS).
- Monitor & Maintain Documentation: Review MDS and care plan documentation, ensuring accuracy, completeness, and chronological order for all resident files.
- Utilize MDS Software Proficiency: Maintain expertise in MDS-related software and ensure accurate data entry and reporting.
- Facilitate Medicaid & Interdisciplinary Meetings: Attend and coordinate Medicaid meetings, ITTM meetings, and care planning discussions to ensure seamless communication and updates.
- Update & Implement Care Plans: Ensure care plan accuracy and update as needed based on resident status and assessment results.
- Provide Quality Indicators Reporting: Generate and distribute Quality Indicators Reports for DON, Administrators, and relevant staff to drive quality improvement initiatives.
- Manage MDS Data Transmissions: Perform timely and accurate MDS submissions in compliance with reporting standards.
- Stay Current with Industry Standards: Attend workshops and training sessions to remain updated on industry changes and best practices.
- Oversee Ancillary Management: Collect, manage, and report ancillary services data to corporate billing personnel.

- Holds a current Nursing License in good standing.
