The Field Supervisor provides service/care coordination of home and community based, one on one support to children and youth with functional limitations and cognitive impairments. The Field Supervisor also provides Behavioral Health Professional ("BHP") training and supervision. Responsible for ensuring COR Health's compliance with all aspects and levels of Federal, State and local laws, regulations and standards.
About this Section 28 Field Supervisor job:
- Explain and complete the necessary documentation with families as required by MaineCare regulations; adhere to all Section 28 rules and regulations
- Completes the necessary documentation with families as required by COR Health policy
- Coordinate and participate in regular treatment team meetings and Individual Treatment Plan reviews; incorporate staff when appropriate / needed
- Works directly with children, families, BHPs, and mental health treatment teams in creating and updating treatment plans; identify and interpret treatment priorities and develop a service plan outlining goals, measures, methods, and discharge plan
- Respond to meeting invitations and attend treatment appropriate community based meetings
- Be sensitive to and strive to understand client’s diagnoses and physical/cognitive/emotional status; engage in continuous education
- Work with staff and parents/guardians/families/natural supports in transferring effecting teaching strategies
- Provides training, support, and supervision to BHPs; provide monthly supervision in person, over the phone, and / or home or community based; and provide performance feedback
- Explain and teach the Individual Treatment Plan; implement as needed, following objectives/goals and methods as written
- Collect data based on goal work; complete accurate and objective communication notes; submit all required documentation to supervisor / payroll / billing in timely manner per company policy
- Fulfill all duties and responsibilities as a Mandated Reporter; report all suspicions of abuse, neglect, and/or exploitation; support staff as needed in fulfilling their role
Depending on the transition to a full time caseload, you may also have the opportunity to work as a Behavioral Health Professional.
COR Health's Behavioral Health Professional (“BHP”) implements an individual treatment plan (ITP) one on one with a client (up to age 21) on a weekly basis. An ITP may include goals on behavior management, safety, and social, independent living, and daily living skills. BHPs must document details regarding the goals that are worked on, parental involvement, methods, and the status of skill retention and development.

EMPLOYMENT REQUIREMENTS:
* 4 Year College Degree in Related Field and 2 years working with children.
* Satisfactory Criminal Background, Sex Offender Registry and Child Protective Checks
* Valid Driver's License is required, reliable transportation, automobile registration and insurance
* 2 forms of government issued IDs
* Eligible to work in the United States
