Physical Therapy Team Member

Responsibilities:
- Develop and implement individualized treatment plans for patients with diverse orthopedic and neurologic diagnoses.
- Work collaboratively with multidisciplinary teams to ensure seamless care coordination and transition planning.
- Maintain accurate and detailed records of patient evaluations, treatments, and re-evaluations.
- Stay current with industry developments and best practices through ongoing education and training.
Requirements:
- Diploma from a CAPTE-approved Physical Therapy Program.
- Current state license to practice physical therapy or in process.
- Minimum 1 year of experience working in a physical therapy setting.
