We have an immediate opportunity for an Attendance Case Manager to join our Community Services team at Disston Elementary School.
This position is full-time and on site at Disston Elementary School
Minimum starting pay - $21.40/hr (approx. $44,500/yr)

In addition, as a Attendance Case Manager your duties will include:
- Providing case management services and support to young people and their families for whom risk factors for truancy have been identified; managing referred cases in collaboration with the school prior to or during the SAIC/SAIP process to remove barriers to school attendance and to slow or stop the accrual of absences, ultimately reducing the number of referrals to Regional Truancy Court; advocating with and for clients for access to appropriate level of care placements
- Working collaboratively with school partners to create a positive school culture that celebrates attendance and encourages regular school involvement including developing and implementing a school-wide incentive plan
- Engaging students/families in a collaborative relationship that will enhance the potential for the student’s completion of service with positive outcomes; completing weekly contact with families on assigned caseload to support with social services and health navigation; conducting regular home visits as needed to assess families’ needs for services and develop a service plan to remove barriers to regular school attendance
- Working as a member of the Integrated Case Management team; participating in SDP Attendance Team or Multi-Tiered System of Supports and working closely with schools, Community School Coordinators and other School Partners
- Adhering to CORA and regulatory entity guidelines concerning caseload requirements, service delivery, accountability and confidentiality; maintaining accurate and timely documentation of services provided to students/families; participating in all necessary interdisciplinary functions and meetings; adhering to training requirements established by CORA or other regulatory entities; attending regular supervision and department meetings
- Proactively seeking and maintaining awareness of community resources and facilitating student/family access to appropriate resources and services necessary to support and supplement service goals
- Completing other tasks, projects and assignments as directed
MINIMUM QUALIFICATIONS:
- Bachelor’s (or master’s) degree in social work, human services or related discipline
- 2 years’ work experience in a social services capacity; treatment or intervention services experience preferred
- Working knowledge of delinquency prevention concepts and the effects of substance use and abuse
- Demonstrated case management experience preferably with young people and families
- Outstanding communication and interpersonal skills; ability to establish and maintain positive, productive working relationships internally and with those external to CORA (students, families, teachers, school administration, etc.)
- Valid driver’s license required
- Proficient with Office 365 applications
