What You'll Do :
As a key member of our Social Services Department, you will play an integral role in planning, organizing, and directing the overall operations of social services across our facilities. Your work will directly contribute to the well-being of our residents, ensuring they receive the best possible care and support. You'll be responsible for :
Conducting comprehensive assessments and developing personalized care plans for residents.
Facilitating safe transitions back into the community through expert interdisciplinary discharge planning.
Advocating for residents' rights and ensuring their dignity is upheld.
Collaborating with medical and nursing staff to address the psychological and emotional needs of residents.
Leading efforts in advance care planning and ensuring residents' wishes are respected.
What We’re Looking For :
Education : Bachelor’s degree in social work or a related human services field (sociology, gerontology, psychology, etc.).
Experience : Minimum of one year of supervised social work experience in a healthcare setting, with specific expertise in Long Term Care, care planning, and discharge processes.
Skills : Strong communication, advocacy, and organizational skills. Ability to work independently and as part of a team.
