What are the responsibilities and job description for the Disaster Case Manager position at CORE Community Organized Relief Effort?
The Case Manager will conduct intakes, refer and assist clients to resources and public assistance programs, and assist disaster impacted individuals in their recovery process. This is a full-time 3-month long position with the possibility of extension.Key ResponsibilitiesServe as a case manager for households impacted by disasters, taking on a large client load and assisting them with dynamic recovery-based needsContact potential clients from outreach efforts to encourage participation and enroll if appropriateConduct client intakes including collecting key information in a sensitive and nonjudgmental mannerDevelop trust and rapport with clients and refer to programs and resources as neededAssist clients with applications to public assistance programs such as Cal Fresh, Medi-Cal, etc.Work closely with program staff to provide a continuum of servicesAdhere to all program guidelines, policies, protocols and workflowsUpdate and shares community resources with program staffTrack and monitor program data and deliverablesSupport staff members in training and capacity building on case management and resource navigation servicesOther duties as assignedTravelTravel within Los Angeles County, including to CORE's headquarters in the City of Los Angeles' Chinatown DistrictQualificationsBachelor's degree in related filed or two years' experience in resource navigation or equivalent experience>
Experience : Minimum 1-2 years' in experience in resource navigation and / or case management, preferably in the public or non-profit sectorDemonstrated experience working families from diverse age, cultural, ethnic and socioeconomic backgroundsDemonstrated experience with data management and coordinationDemonstrated experience in identifying and developing partnerships with organizations to meet needs of low-income clientsDemonstrated experience in health and / or nutrition programs a plus.Skills and Competencies : Ability to work independently and as part of a teamAbility to empathize and establish supportive rapport with clients while maintaining professional boundariesExcellent written and verbal communication skillsProficient in MS Office applications and referral platformsWork proactively with little supervisionFlexible and adaptive to a frequently changing environmentClose attention to detail and a high level of accuracyExperience working with FEMA Individual Assistance programs highly preferred Bilingual in English and Spanish or another language highly preferred$27.81 - $27.81 an hourCORE Southern California is an equal opportunity employer and encourages candidates from diverse backgrounds to apply.
