About the Role
The Hospice Field Case Manager RN will work closely with an interdisciplinary team to provide symptom management and end-of-life care to patients within their homes.
You will assess, plan, implement, coordinate, monitor, and evaluate the strategies and services required to meet the patients/family's end-of-life care needs.
This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
