About the Unit
Our Primary Care Clinic is located at 2770 East Beltline Ave Northeast in Grand Rapids, serving the needs of patients across Michigan. As a Triage LPN, you will work collaboratively with our multidisciplinary team to assess, plan, implement, and evaluate patient care.
You will inform patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate. You will also contribute to the documentation of patient care via computer/written patient record in a manner consistent with guidelines.
Our clinic offers a hybrid work model, with opportunities for remote work after successful completion of orientation and training. We are seeking a full-time Triage LPN who is passionate about delivering high-quality care and is eager to join our dynamic team.
Job Description:
- Under the direction of the RN/Advanced Practice Provider/Physician and in collaboration with other members of the healthcare team, contributes to the assessment, planning, implementation, and evaluation of patient care
- Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate
- Documents care provided via computer/written patient record in a manner consistent with guidelines
