We are a skilled nursing team providing home care services in the West-Central areas of Michigan. Our team is dedicated to delivering high-quality patient care, and we are currently seeking experienced RN's, LPN's, and New Grads to join our team.
Our Mission
We strive to provide personalized care close to home, enabling our nurses to visit between 5-7 patients per shift. From Ludington to Grand Rapids, we cover a wide area to ensure our patients receive outstanding care.
Job Responsibilities
- Under the direction of the RN/Mid-level Provider/Physician, contribute to patient care assessments, planning, implementation, and evaluation.
- Inform patients and families about treatments, medications, and related activities.
- Document care provided via computer/written patient record.
