Post Acute Care Units
A Wing is a 58-bed Medicare/Medicaid unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac related issues needing monitoring. Nurses work closely with a variety of healthcare professionals including doctors, social work, dieticians, PT/OT, hospice, and spiritual support.
B Wing is a 40-bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. B wing is a great unit for nursing students to develop their organizational skills and gain some experience in dealing with challenging behaviors. This unit also works closely with a multidisciplinary team meets and discusses the resident's plan of care.
C Wing is a 58-bed Medicare/Rehabilitation and Skilled unit and is a great place to learn basic med-surg, I.V. therapy & wound treatments nursing skills. Nurses on this unit along with a host of healthcare professionals work to provide and implement the best treatment plan for residents. This fast-paced and busy unit provides a great atmosphere for PT/OT residents to be motivated and reach their goals.
About the facility
We are a long-term care facility with 2 floors, 4 units, providing care for post-acute rehab patients. The first floor has 96 beds and the second floor has 100 beds. Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long-term care, and rehab residents. With 196 beds, we accept patient specialties such as general rehab, neuro-rehab, cardiac, respiratory, and cancer. We also have various programs for residents to participate in.
Job Summary
As a Licensed Practical Nurse (LPN), you will contribute to the assessment, planning, implementation, and evaluation of patient care under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the healthcare team.
- Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician.
- Documents care provided via computer/written patient record in a manner consistent with guidelines.

This job requires a graduate of an approved LPN program, experience in Skilled Nursing/Long Term Care, and licensure in the State of Michigan. Basic Life Support (BLS) certification from the American Heart Association is also required.
