About Our Post Acute Care Units
We have three units: A Wing, a 58-bed Medicare/Medicaid unit with residents requiring close monitoring for conditions such as CHF, dehydration, cellulitis, diabetes, DVT, and stable cardiac issues. Nurses collaborate with healthcare professionals like doctors, social workers, dietitians, PT/OT specialists, hospice staff, and spiritual support teams.
B Wing is a 40-bed Medicare/Medicaid/Dementia unit where we provide care to residents with various diagnoses. This unit is ideal for nursing students seeking to develop organizational skills and experience in managing challenging behaviors.
C Wing is a 58-bed Medicare/Rehabilitation and Skilled unit offering opportunities to learn basic med-surg, I.V. therapy, and wound treatment nursing skills. Healthcare professionals work together to implement effective treatment plans for residents.
About Our Facility
Our facility has two floors with four units, providing care for long-term care residents and post-acute rehab patients. The first floor has 96 beds, and the second floor has 100 beds. Nurse-patient ratios vary depending on shifts and patient populations. We cater to dementia residents, long-term care patients, and those undergoing rehabilitation.
We accept patients with specialties such as general rehab, neuro rehab, cardiac, respiratory, and cancer. Various programs are available for residents to participate in.
Licensed Practical Nurse (LPN) Role
Under the guidance of RNs, mid-level providers, or physicians, LPNs contribute to patient care assessment, planning, implementation, and evaluation. They inform patients and families about treatments, medications, and related activities, working in coordination with healthcare teams. LPNs document care provided via computer or written records, following guidelines.
Required Qualifications
Graduate of an approved LPN program
Experience in Skilled Nursing/Long Term Care
LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire required
CRT-Basic Life Support (BLS) - AHA American Heart Association Upon Hire required
