Scope of Work
Our Mental Health Professionals plan and prioritize care for individuals and populations, focusing on strategies that promote optimal health within populations. They provide peer support and role modeling related to life skills and health/wellness during home visits, if applicable.
The scope of work involves:
- Assessing internal referrals and providing individual, short-term therapy services or care management services and monitoring patient progress.
- Developing plans of care and making recommendations to PCPs, specialists, and other healthcare team members regarding mental health treatment strategies and identifying strategies to maximize continuity of care across the continuum.
- Assessing and providing follow-up for the Systematic Case Review process, which includes consultation with a psychiatrist and serving as liaison between PCPs and consulting psychiatrists to assist in psychotropic medication management for patients.
- Responding to patients' urgent mental health concerns, including suicide risk assessments, and assisting patients in accessing appropriate levels of care.

Qualifications
To be successful in this role, you will need:
- 2 years of relevant experience in a related field.
- LMSW - Master Social Worker, MI Licensed, upon hire required. We will consider LLMSW candidates.
Work Environment
This is a full-time position with a standard schedule of Monday to Friday, 8:00 a.m. to 5:00 p.m., with 40 hours per week.
