Job Description:
The Therapy Care Provider will be responsible for assessing patient needs and developing treatment plans to promote optimal recovery. This includes evaluating patient disability, documenting patient information, and training staff on patient needs.

Requirements:
A Bachelor's Degree or equivalent from an accredited physical therapy program is required. The shift is Monday to Friday and every fourth weekend.
