About the Job
This Home Hospice Care Professional role is responsible for providing end-of-life care and symptom management to individuals with a life expectancy of six months or less.
Key Responsibilities:
- Assess patient needs and develop individualized plans of care
- Implement and coordinate patient care services
- Monitor and evaluate patient progress
- Provide education and support to patients and families
Qualifications:
- Associate's Degree in Nursing
- 1 year of relevant experience, preferably in hospice or related field
- Registered Nurse (RN) - State of Michigan required
- BLS through American Heart Association or American Red Cross within 90 Days required
