What are the responsibilities and job description for the Weekend RN Program Lead position at Corewell Health?
Corewell Health is seeking a skilled Registered Nurse to join our Weekend Program team. As a nurse in this program, you will provide end-of-life care and symptom management to individuals with a life expectancy of six months or less. Our weekend program consists of working 48 out of 52 weekends, allowing you to have a stable schedule and work-life balance.
Job Responsibilities:
- Assess, plan, implement, coordinate, monitor, and evaluate the strategies and services required to meet the patients'/family's end-of-life care needs
- Develop and implement individualized plans of care that address the patients'/family's needs
- Collaborate with interdisciplinary teams to provide comprehensive care
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements:
- Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
- 1 year of relevant experience, preferably in hospice or related field
- Registered Nurse (RN) licensure in the state of Michigan
- Basic Life Support (BLS) certification through American Heart Association or American Red Cross within 90 days
