Our team is growing, and we’re looking for a professional and empathetic Care Coordinator to lead our team of Engagement Partners.
Job Summary
As a Care Coordinator, you will supervise a team of Engagement Partners “EPs”. These EPs provide social and emotional engagement to older adults in their homes or community-based settings. They take a holistic approach to wellness, while enhancing their client’s quality of life and overall well-being. Care Coordinators are responsible for scheduling and providing ongoing support and supervision for EPs.
Care Coordinators work closely with clients, families, and team members to design solutions, provide support, and coordinate care that enables optimal quality of life outcomes. This role requires someone who is solution-driven, highly organized, emotionally intelligent, and deeply committed to forming meaningful, respectful relationships.
What You’ll Do:
- Manage Engagement Partner’s Performance to Ensure Work Meets Quality Standards
- Oversees EPs' execution of the
- Ensures that EPs are using person centered practices and principles that enhance quality of life for clients served.
- Takes Personnel Actions
- Collaborates with Talent Management in hiring interviews for EP positions.
- Takes disciplinary actions consistent with Corewood’s policies and procedures to address performance concerns.
- Conducts annual performance evaluations for EP team members.
- Monitors, Documents and Audits
- Monitors and reviews care notes, timesheets, mileage and billing on a daily basis.
- Monitors and reviews EPs' company debit card purchases made on clients' behalf.
- Updates and maintains supervision records and performance evaluations for developmental feedback and performance evaluations.
- Facilitates/Participates in Meetings to Collaborate with Team Members
- Facilitates regular EP and inter-departmental meetings.
- Participates in weekly organizational and program leadership meetings to stay informed of changes affecting policies, procedures and practices.
- Participates in and/or facilitates work groups and committees as assigned.
- Scheduling
- Manages EPs' schedules through collaboration with team on optimal client/EP matches.
- Manages call-outs and approves time off requests, while prioritizing client scheduling needs
- Optimizes scheduling to ensure client needs are met, while simultaneously meeting EP Performance Utilization (PU) benchmarks
- Client Information, Services & Follow-up Activities
- Documents and updates of the client’s current information include physical and mental status, social circumstances, environment, and overall well-being.
- Enhances the quality of life of people supported by recommending and arranging meaningful social, spiritual and cultural activities.
- Identifies and addresses any service gaps and coordinates with Corewood Home Care and Care Management team on potential solutions.
- Develops and implements person centered solutions to resolve client and EP concerns.
Qualifications:
Required:
- Bachelor’s or master’s degree in human services, healthcare, or hospitality. MSW is highly desired.
- A minimum of 2 years’ experience interacting directly with clients in a customer-service centric environment
- Strong communication and interpersonal skills.
- Reliable transportation, valid driver’s license, and willingness to travel to the office.
- Must pass background check and drug screening.
Preferred:
- Experience working with older adults, families, or individuals with complex needs.
- Background in care coordination, team leadership, or social services.
