The Davis Community requires that all current and new employees, including contract staff, receive an annual influenza vaccination and TB skin test unless a reasonable or disability accommodation is granted.

The Social Worker is responsible for assessing and coordinating social services. Social services will include psychosocial needs, resident advocate when working in conjunction with the family / responsible party and the interdisciplinary team. Completes all documentation regarding MDS / care plans / RAI in designated household and supports other guides in this task across multiple households.

MAJOR WORK ACTIVITIES :

1) Conducts initial psychosocial assessments, social histories, and mini-mental status exams.

2) Performs timely assessments, to include charting of interventions or changes in the resident care plan.

3) Responsible for the administration of Social Work component of the MDS.

4) Participates upon request in Care Plan meetings in any household.

5) Serves as the Social Worker mentoring Household Coordinators across all households.

6) Creates community through maintaining “home” in the physical environment, maintaining a sense of welcoming through elimination of institutional barriers, enhancing relationships in the household through shared activities of living, promoting compassionate services among the team by offering emotional, psychological and spiritual support to all.

7) Determines needed support services residents may require to promote their well-being. Facilities the effective completion of MDS / care plans / RAI documentation with regards to psychosocial well-being, social service and activity needs.

8) Promotes the residents' well-being and satisfaction by supporting resident-centered care, developing and interpreting standards of care, guiding the interdisciplinary team as it establishes and implements the residents’ plan of care with the resident, family or responsible party and the household team.

9) Complete all clinical chart documentation according to best practice at time service rendered, prior to end of current shift, and / or by “late entry” with date, time, and legible signature or initials where appropriate. Documentation that is not time specific / shift specific, i.e. such as MDS, Care Plans, and certain summaries may be completed with signature and date on the day of documentation, completion and / or at time of review.

10) Keeps abreast of current state and federal regulations, as well as professional standards and makes recommendations for incorporation in to policy and procedure.

11) Support management / administration decisions through periods of change or stress.

12) Point of contact for psychiatric support services.

13) Provides emotional, psychological support to residents and family member / responsible parties to include but not limited to facilitating support groups, group settings to improve social functioning, mental health services and advocates on behalf of the resident.

14) Responsible for resident discharges as follows : coordinating with family member / responsible party and provide appropriate referral resources in the community.

15) Maintain knowledge in the following areas : department policies and procedures, Resident Hand Book, Resident Rights, Social Work Code of Ethics published by NASW, and legislative changes that may affect the residents, this includes changes in Medicare, Medicaid, and Social Services.

Requirements :

Education : Bachelor’s degree in social work, or human services field including but not limited to sociology, psychology special education, rehabilitation counseling.

Licensure / Certification : Required : CNA Certification : certified / seeking certification

Experience : 3 - 5 years working experience in a skilled nursing or retirement community preferred. Specialized training or experience in geriatric activities, social work or gerontology is desirable.
