Essential Duties And Responsibilities
- Provides consistently excellent medical care, practic ing appropriate management through us e of sound clinical judgment , best practice standards and evidence - based medicine.
- Consults with other staff or specialists to make appropriate referrals for acute, chronic and preventive health care, as may be indicated .
- Coordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary.
- Educates patients on preventive health care and chronic disease management.
- Treats patients and staff respectfully, courteously, appropriately.
- Maintains confidentiality in regards to patients, staff and program matters at all times.
- Maintains safe and clean working environment by complying with procedures, rules and regulations.
- Protects clients and employees by adhering to infection-control policies and protocols.
- Completes appropriate charting of notes and coding , r esponds to patient and colleague requests in a timely and efficient manner.
- Attends/ participates and contributes to medical staff/departmental meetings.

- APRN : A nurse practitioner must have an MSN degree and current CT RN/APRN licensure.
- PA : Graduate from an accredited Physician Assistant program. Current certification by the National Commission on Certification of Physician Assistant (NCCPA).
- BLS certification is required .
