Client Care Coordinator

Job Summary

As a Client Care Coordinator, you will be responsible for reporting to and assisting the Assistant Operating Director in providing operative support. The Client Care Coordinator’s primary focus will be to complete all new client assessments for all onboarding individuals beginning services with Cornerstone Caregiving, along with all reassessments annually for current clients. This position is required to enter all information, with detail, into the company’s platform.

Roles and Responsibilities

• Assure all interactions with potential and current clients are conducted in a

courteous and professional manner.

• Assist with helping to follow-up on outstanding leads or potential clients.

• Conduct assessments with potential new clients.

• Conduct annual re-evaluations of current clients.

• Complete all documentation required in the AxisCare system.

• Help with marketing events (parties, special events, etc).

• Provide general operational and administrative support to the Assistant

Operating Director.

Required Skills

• Outstanding interpersonal and communication skills.

• Excellent verbal and written communication, including ability to present in a clear

and concise manner.

• Proficient computer skills, including Microsoft Office and Google Docs.

• Detailed-oriented with strong organizational and time management skills.

• Ability to multitask and work at a steady pace.

• Ability to split after-hours on-call requirements of the job with the Cornerstone

Team.

• Strong ethics.

• Ability to clear background checks.

• High school diploma or equivalent.

Physical Requirements

• Prolonged periods of sitting in the car and with clients.

• Ability to lift 25 pounds without assistance.

Job Type: Part-time

Pay: From $15.00 per hour

Expected hours: 20 per week
