What are the responsibilities and job description for the Patient Care Coordinator position at Cornerstone Caregiving?
We are seeking a Client Care Coordinator to join our team and provide excellent customer service to our clients. The Client Care Coordinator will be responsible for managing client accounts, responding to inquiries, and providing general administrative support. The ideal candidate has a strong customer service background and is able to work independently.
This is a PRN position, so hours can range weekly from 4 hours to 20hrs

Responsibilities and Duties
- Using data to do analytical tasks and charting tasks related to patient information
- Communicating with patients about their status and condition through emails, phone and face-to-face conversations
- Managing patient accounts with contact information and financial information in a confidential manner
- Scheduling patient visits and answer questions, including billing and preparation
- Being able to communicate with other team members to solve client problems
- Perform initial and ongoing in-home evaluation, caregiver introductions, coordination and supervision of client services
- Be able to assist in filing, coordinating and carrying out day to day operations support to office staff
Qualifications and Skills
- Must possess and maintain a Valid Driver’s License and maintain adequate auto insurance. Traveling Between Trumbull, Mahoning, Columbiana Counties with potential of Portage County and Jefferson County
- Ability to write reports and business correspondence
- Ability to effectively present information and respond to questions from team members, clients, customers, and the general public
- Ability to evaluate situations and make complex decisions using critical thinking and reasoning
- Pass a background check

Pay: $15.00 - $18.00 per hour
