Duties:
- Provide personal care assistance to patients, including bathing, dressing, and grooming.- Assist patients with daily living activities, such as meal preparation, medication management, and household chores.- Monitor and record patients' vital signs and medical conditions.- Assist patients with mobility and transportation.- Provide emotional support and companionship to patients with dementia or other cognitive impairments.- Maintain a clean and safe living environment for patients.- Maintain accurate and up-to-date medical records.- Adhere to HIPAA guidelines and maintain confidentiality.- Perform catheter care and other specialized care tasks as necessary.- Work collaboratively with the patient's healthcare team to ensure comprehensive care.
Qualifications:
- Current certification as a Home Health Aide and CNA- Experience working in a home care or assisted living setting.- Familiarity with long-term care and memory care principles.- Compassionate and empathetic nature.- Strong communication and organizational skills.- Ability to lift 50 pounds or more, as required by the job.- Valid driver's license and reliable transportation.
