Cornerstone Home Health, Inc is seeking an experienced home health RN to join
our team.
Duties and Responsibilities
Meet patients in their homes complete OASIS assessment of all types
and SN Visits. Teaching training and assessing patients. IV therapy, Wound care, Lab
draws, sup visits. Training, as well supervising home health aides. This position will
require the ability to use moderate (30-50lbs) physical exertion on a regular basis.
Comfortable with home health charting. Attending case conference weekly. Keeping
staff up to date with patient needs while in the field. Able to navigate web-based
software. Must be able to use devices in patient homes generating EVV. Current EMR
Axxess. Willingness to travel within the metro area up to 30 or 40 miles from office
location.
Job Type:
Part-time: Expected hours: 5 – 10 per week (some weekends)
Possible full-time
RATE (range depending on recent home health experience)
$35-55 per visit of skilled nursing
$85-105 for OASIS recertification
$110-125 for SOC OASIS
