Cornerstone – Case Manager

Responsibilities of the Case Manager
- Provide high quality mental health case management services to clients
- Coordinate and facilitate referrals to appropriate resources and services
- Develop person-centered treatment plans and recovery-oriented goals
- Advocate for clients' needs and rights
- Provide crisis intervention and support as needed
- Maintain accurate and up-to-date client records
Qualifications of the Rehabilitation Counselor
- Bachelor’s degree in Behavioral Sciences preferred
- Equivalent combination of education and relevant work experience including military service can be substituted
- The use of a personal vehicle to assist clients as needed is required
