We are seeking a skilled and dedicated Physical Therapy Assistant (PTA) to join our team at Cornerstone Rehab. As a PTA, you will be responsible for delivering high-quality patient care and collaborating with our diverse team of clinical professionals.
Responsibilities
- Provide physical therapy services to adult and geriatric patients, tailoring treatment plans to meet individual needs and abilities.
- Prepare and maintain accurate documentation, including progress notes and discharge summaries.
- Develop and implement comprehensive plans of care in accordance with patient goals and capabilities.
- Travel to nearby facilities as required to provide care and support.
Qualifications
- Possess a valid State Physical Therapy Assistant License and Credentials.
- Demonstrate experience working in SNF and/or Acute care settings.
