What are the responsibilities and job description for the Lead Direct Support Professional (24 hour homes) position at Cornerstone Services Inc.?
General Functions: Responsible for carrying out the directives of the Case manager for the daily operation of the client’s home. Assist the Case Manager in the coordination and implementation of client’s Individualized Services Plan/Individual Habilitation Plans.
Specific Functions:
- Participate as a member of the interdisciplinary team in the process of planning clients' Individualized Services Plan/Individual Habilitation Plan.
- Assist the Case Manager in monitoring and implementing Individualized Services Plans/Individual Habilitation Plans to insure client progress in meeting individual goals.
- Provide for skill training for clients in accordance with Individualized Services Plans/Individual Habilitation Plans. Skill training areas may include cooking, meal planning, housekeeping, kitchen chores, laundry, personal shopping, banking, etc. Ensures proper documentation of skill training activities.
- Assist clients in budget preparation and banking procedures.
- Monitor the health status, grooming and hygiene of each client, and schedule prompt intervention from community providers as appropriate.
- Monitor and document client's self-administration of prescribed medication and treatments. Reorder necessary prescriptions from the pharmacy, if applicable.
- Assist in the development of activity schedules along with clients, consistent with Individualized Services Plans/Individual Habilitation Plans.
- Assist the Case Manager and other residential supervisors in the overall operation of the client's home within established agency policies and procedures.
- Function as the liaison to the Case Manager regarding building maintenance issues.
- Communicate to the Case Manager any concerns that might require the services of consultants.
- Responsible for supervision, evaluation, scheduling, selection, training and discipline of assigned staff members.
- Responsible for arranging coverage in the event of staff absences and/or vacancies.
- Responsible for carrying an agency cell phone and responding to calls in the absence of the Case Manager.
- Communicate appropriately and effectively with consultants and other agency staff. Responds appropriately to client's family and friends, as well as guests of the clients.
- Properly complete daily documentation: Progress Notes, Incident Reports, Inspection Checklists, etc.
- Implement behavior development programs and documentation.
- Provide counseling and crisis intervention to clients in need of such assistance and guidance.
- Assist in the implementation of client meetings in accordance with the current policies and procedures, including safety meetings and resident council meetings and forward copies to appropriate agency staff.
- Assist in the implementation of fire drills, safety checklists, disaster drills, and other safety procedures.
- Operate agency vehicle or personal vehicle in accordance with established policies and procedures.
- Respond to emergency situations per established procedures.
- Be familiar with the location and organization of the client record and files and other record keeping systems.
- Monitor and purchase food to assure items match approved menu.
- Maintain an adequate supply of cleaning and hygiene supplies.
- Submit client budgets, grocery, hygiene, and cleaning receipts to Accounting Department at the end of each month.
- Act in accordance with established safety regulations, policies and procedures.
- Perform other related duties as assigned by the Case Manager, Coordinator of Residential Services, Director of Residential Services and/or Vice President/Chief Operating Officer. Maintain ongoing compliance with all agency-related training and certification requirements.
- Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
- Operate agency or personal vehicle in accordance with established policies and procedures.
- Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.
- Other duties as assigned.
Qualifications: Bachelor’s degree in social services related field preferred. Must be at least 18 years of age with a high school diploma or equivalent and at least a minimum 1 year experience in human services with demonstrated ability of leadership. Driving record and auto insurance in accordance with agency policy. A post offer physical and criminal background check required. EOE. Prior supervisory experience is desired. Reliable transportation required. EOE.
