What are the responsibilities and job description for the Medical Appointment Associate position at Cornerstone Services Inc.?
General Functions: Provide services and supports for clients in need of medical care in the greater Joliet and/or Kankakee County communities.
Specific Functions:
- Schedule, as needed, all medical appointments with physicians who accept Public Aid and/or Medicare, if applicable.
- Prepare appropriate paperwork necessary for doctors' appointments.
- Transport persons served to medical appointments.
- At conclusion of doctor's appointment, provide all prescriptions to the RN for processing. Scan the medical report(s) into the person's record, tagging appropriate members of the treatment team.
- Maintain a schedule of all medical annuals and schedule them as they are due. Ensure that all appointments are entered into the Medical Appointment Calendar; update calendar as new appointments are made.
- Obtain new scripts from the physician as they become outdated (six months for all scripts except PRNs which require annual renewal).
- Assist residents to and from psychiatric reviews, if needed.
- When primary staff are unavailable, transport any ill resident from vocational site and supervise when not involved in appointments as directed by Registered Nurse or Coordinator of Medical Services.
- Help other caseloads with above duties, as needed.
- Properly complete daily progress notes and incident reports, as well as any other required documentation.
- Maintain awareness of the physical condition and activity level of people served and report suspected problems or symptoms to QIDP or other administrative personnel.
- Communicate appropriately and effectively with house staff, QIDP, MRS, and staff in other programs and departments. Respond appropriately to family and friends of people served, as well as guests of the home.
- Be familiar with the location and the organization of the residents' records and files, as well as other record keeping systems.
- Provide counseling and crisis intervention to clients in need of such assistance and guidance.
- Be familiar and act in accordance with established safety regulations, policies, and procedures.
- Provide complete documentation of behavioral incidents and carry out resident's behavior programs.
- Communicate with house staff when taking a person served to a medical appointment, providing information regarding time of appointment, as well as pick-up and drop-off times.
- Notify QIDP of residents' medical issues, which may require immediate medical attention, or an appointment be made within a week.
- Perform other unspecified tasks and duties at the direction of the Coordinator of Medical Services, nurses, or any other supervisory personnel.
- Maintain ongoing compliance with all agency-related training and certification requirements.
- Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
- Operate agency or personal vehicle in accordance with established policies and procedures.
- Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.
- Other duties as assigned.
Qualifications: Must be at least 18 years of age and have a high school diploma or equivalent. Driving record and auto insurance in accordance with agency policy. A post-offer physical and criminal background check required. Reliable transportation required. EOE.
