What are the responsibilities and job description for the Post-Transition Care Manager position at Cornerstone Services Inc.?
General Functions: Serve as an integral member of the agency's Williams Transitions Team, providing post-transition care to individuals (Class Members (CM)) who have moved from the nursing home into the community.
Specific Functions:
- Conduct post-transition visits in-person and telephonically, weekly for the first month of transition and then monthly for the following 18 months. Travel may include longer distances within the state of Illinois.
- Arrange and coordinate required Comprehensive Service Plans (CSP) within specified timeframes, ensuring accurate tracking with the excel spreadsheet.
- Utilize the CSP process to identify all supports and services that are needed for continued success in the community.
- Update all CSPs every 180 days and provide a copy to the class member (CM), case manager, and any other supportive party the CM may want as their support.
- Complete all weekly and monthly notes as required by the grant in the Webapp and internal system.
- Maintain IM CANS certification and complete re-assessments every six months as needed to CM? that are being served outside of Cornerstone Services.
- Amend class members' IM CANs upon transition into the community as needed.
- Coordinate completion of the CCMTP Housing Satisfaction Survey, ensuring class members have the necessary tools to complete the paper or online survey.
- Engage in weekly team meetings to provide updates on CMs' progress.
- Assist with secondary moves and/or moving CMs to an SRN/811 unit when available.
- Provide check-ins to any CMs who need a respite stay at a SMHRF and/or check-in with CMs during their first 90 days of a readmission to a SMHRF.
- Assist with any reportable incident or follow-up that is needed in the event our RN is not able to do it in the required timeframe.
- Provide support and resources to CMs when requested and work with the CRSS to identify CMs who need additional supports in order to be successful in the community.
- Maintain a driving record and automobile insurance in compliance with agency policy.
- Meet or exceed assigned weekly billing quotas as determined by the Director of Professional Services.
- Participate in all agency and department training courses, meetings, online trainings, and any additional recommended training or meetings as required.
- Ensure compliance with all certification requirements, including First Aid/CPR, Safety Care, etc.
- Maintain ongoing compliance with all agency-related training and certification requirements.
- Follow established procedures to respond to emergency situations, including medical, behavioral, fire, severe weather, and other hazards.
- Operate agency or personal vehicles in accordance with established policies and procedures.
- Own, maintain, and use personal smartphones for company business in accordance with established policies and procedures.
- Maintain reliable transportation to get to work timely.
- Perform any other duties as assigned.
- Maintain ongoing compliance with all agency-related training and certification requirements.
- Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
- Operate agency or personal vehicle in accordance with established policies and procedures.
- Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.
- Other duties as assigned.
Qualifications: Qualified Mental Health Professional preferred (master's degree in counseling, social work, or related field with practicum and 2 years of experience under the supervision of a Qualified Mental Health Professional). Qualifications consistent with a Mental Health Professional will be considered in conjunction with prior clinical experience. CADC and DASA experience strongly preferred. Driving record and auto insurance coverage in compliance with agency policy. A post-offer physical and criminal background check required. EOE.
