Job Summary
The QDDP Program Manager serves as the primary leader of the residential support team. They are responsible for developing, writing, and overseeing the implementation of Individual Support Plans and writing Quarterly Person-Centered Reviews. We are looking for driven, dependable, and compassionate individuals who want to make a difference in people’s lives. You will assist adults with developmental disabilities through oversight of many residential activities and community integration. This job can offer great experience for anyone interested in the human services field and the fulfillment of being a part of Cornerstone’s mission.
Examples of Program Manager responsibilities
- Performs on an on-call function
- Reviews assessments to identify the supports and services needed to address residents' goals and desired outcomes
- Participates in shared planning by assisting residents in identifying and communicating their needs and requests for services and supports
- Monitors documentations, financials, records and coordination of services for residents
- Conduct and facilitate client intake and admissions
- Conduct inspections and ensure compliance with agency and licensing regulations
- Trains staff on new and changing policies/procedures to ensure correct and timely implementation

- Bachelor's Degree OR 5 yr. of relevant experience
- Valid drivers license
