About the Role
As a Community Liaison, you will play a vital role in coordinating and facilitating referrals to our Home Care, Hospice Care, Life Care, Palliative Care, and Community Care programs. You'll be responsible for outreach efforts to hospitals, healthcare organizations, and community providers, ensuring smooth transitions to home care services for patients and families.
Key Responsibilities
Build and maintain relationships with hospitals, physician practices, skilled nursing facilities, and senior service providers to increase referrals and admissions.
Educate healthcare professionals and the community about Cornerstone VNA's services.
Assist with discharge planning, ensuring patients have a smooth transition to home care.
Serve as a patient advocate, meeting with patients and families to discuss care options.
Collaborate with case managers and clinical teams to ensure appropriate patient care.
Represent Cornerstone VNA at community events, workshops, and educational presentations.
Support business development initiatives to expand service awareness.

What We're Looking For
Education & Licensing: BA/BS degree required; Nursing License (NH or ME), Medical social work or other healthcare professional certification preferred.
Experience: 1-2 years in case management/community liaison roles preferred, plus 3-5 years in healthcare business development or marketing.
Skills: Strong communication, relationship-building, problem-solving, and adaptability.
Technology: Comfortable using computers, tablets, and other office equipment.
