We’re looking for a Director of Social Services who brings heart, leadership, and vision to Coronado Ridge Skilled Nursing and Rehab Center . What You’ll Do:
Resident-Centered Care:
- Empower Residents: Conduct in-depth psychosocial assessments to craft care plans that enrich lives and foster dignity.
- Provide Support: Be the calming voice, the listening ear, and the advocate for residents and families as they navigate challenges.
- Make an Impact: Address adjustment, grief, and end-of-life concerns with sensitivity and respect.
Family & Community Collaboration:
- Bridge Connections: Serve as the trusted liaison between residents, families, and our care team.
- Facilitate Growth: Lead family meetings to resolve concerns, share updates, and celebrate progress.
- Expand Resources: Partner with local organizations to offer innovative programs and resources for residents.
Discharge Planning Excellence:
- Smooth Transitions: Coordinate seamless discharges that set residents up for success—whether returning home or moving to another level of care.
- Teamwork First: Collaborate across departments to develop discharge strategies that meet every need.
Leadership & Compliance:
- Lead Boldly: Guide, mentor, and inspire your social services team to deliver excellence.
- Raise the Bar: Develop creative programs that enhance the quality of life for all residents.
- Stay Ahead: Ensure compliance with state, federal, and CMS regulations while maintaining meticulous documentation.
What You Bring:
Education & Credentials:
- Bachelor’s or Master’s in Social Work (BSW/MSW) or related field.
- State Social Work licensure (LMSW or LCSW) preferred or required based on local guidelines.
Experience & Skills:
- Proven Expertise: At least 2-3 years of experience in social services, ideally in long-term care or a SNF.
- Leadership Flair: Previous supervisory or management experience is a big plus.
- Heart & Strategy: A unique blend of empathy, problem-solving, and organizational skills to balance resident care with compliance.
- Excellent Communicator: Stellar interpersonal and conflict-resolution abilities that build trust and rapport.
