Job description
Correctional Psychiatric Services (CPS)- is a leader in providing healthcare in the MA County Correctional System and is hiring a full time (day shift) Discharge Planner at the Suffolk County House of Correction. The Discharge Planner is responsible for discharge planning for patients receiving medical/mental health services, in accordance with policy and procedure. The discharge planner must facilitate applications of eligibility for Masshealth assistance, as well as coordination of care with outside providers for patients planned release from the facility.
