Corry Counseling of LECOM Health has immediate openings for full-time (37.5 hrs/week) Blended Case Manager (BCM) positions working with adults in the Corry, Erie and Warren County areas. This position is primarily home and community based.
The BCM supports the physical and behavioral health needs of individuals by identifying strengths and accessing resources that enhance the client’s quality of life. Additionally, the BCM collaborates with and monitors other service deliveries to ensure the services are providing what individuals need. The BCM has a monthly productivity goal for services rendered and is held accountable to maintain this goal. This position reports to the Manager of Blended Case Management Services.
Responsibilities:
- Support the physical and behavioral health needs of individuals by identifying strengths and accessing resources that enhance the client’s quality of life, including but not limited to housing, behavioral and physical health needs, benefits, education and employment, and linking with community resources.
- Collaborate with and monitor other service deliveries to ensure the services are providing what families need.
- Work non-traditional hours and provide rotation on-call services.

Requirements:
- Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education.

- Registered nurse
OR
- A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences and two years’ experience in direct contact with mental health consumers.
Additional Requirements:
· Must have a valid driver’s license, current insurance coverage and consistent access to a reliable vehicle.
· Act 33 (PA Child Abuse) must be received prior to hire.
· Act 34 Clearance (PA State Police Record Check) OR the Department of Human Services FBI Fingerprinting must also be received prior to hire along with the Act 33 clearance.
· Mandated Reporter and Safety Care Training within 30 days of hire.
