JOB SUMMARY
The Certified Nurse Assistant provides essential personal care and support to patients within their homes, following guidance and supervision from a registered nurse (RN). This role ensures compassionate, respectful care in alignment with established care plans and patient needs.
RESPONSIBILITIES
Adhere to agency policies and procedures to ensure the highest standards of patient care.
Provide assigned personal care tasks, including bathing and hands-on support, with compassion and professionalism.
Maintain timely and accurate documentation for each visit, in compliance with agency standards.
Communicate any changes in patient condition to the supervising RN promptly.
Assist with household tasks essential to patient health and well-being, as assigned.
Support patient mobility and physical activity as directed, including ambulation and exercise.
Perform basic therapeutic procedures, such as range-of-motion exercises, under supervision.
Assist with patient medications as directed for self-administered medications.
Participate in required in-service training and continuing education to maintain skills.
Engage in patient care conferences and collaborative discussions as scheduled.
Requirements : QUALIFICATIONS
Completion of a Medicare-approved training program for Home Health Aide and / or Hospice Aide, with demonstrated competency.
Compassionate and respectful attitude towards the care of the elderly and those who are ill.
Proficiency in following directions, as well as reading and writing.
Emotional maturity and resilience to meet the demands of patient care.
WORKING ENVIRONMENT
Primarily works in patient homes with occasional tasks completed at the agency office. Requires travel between patient locations.
JOB RELATIONSHIPS
Supervised by : Director of Clinical Services, Clinical Manager, or assigned registered nurses (RNs), physical therapists (PTs), occupational therapists (OTs), and speech-language pathologists (SLPs).
RISK EXPOSURE
High exposure risk, with adherence to safety protocols essential to ensure patient and caregiver protection.
LIFTING REQUIREMENTS
This role requires the ability to engage in physical activity for extended periods, including standing, bending, and lifting as needed for patient care.
