JOB SUMMARY
The Licensed Practical / Vocational Nurse (LPN / LVN) is responsible for delivering skilled nursing care to patients on an intermittent basis in their place of residence. This care is provided under the direction and supervision of a Registered Nurse (RN), in accordance with physician orders and a personalized plan of care. All services are performed in alignment with agency policies and regulatory standards.
QUALIFICATIONS
- Graduate of an approved school of practical or vocational nursing, with an active LPN / LVN license in the practicing state.
- Minimum of one (1) year of nursing experience preferred.
- Commitment to the philosophy and goals of the agency.
- Ability to exercise initiative and make independent judgments as needed.
- Strong communication skills and compassionate approach to patient care.
Requirements : RESPONSIBILITIES
WORKING ENVIRONMENT
Primarily works indoors, both in an office setting and in patient homes, with travel required between patient homes.
JOB RELATIONSHIPS
Reports to : Director of Clinical Services, Clinical Manager, or Registered Nurses (RNs)
RISK EXPOSURE
High risk due to potential exposure to infectious diseases and patient interactions in home settings.
LIFTING REQUIREMENTS
The role requires the ability to engage in physical activities essential for patient care, including extended periods of standing and frequent bending, lifting, and heavy lifting on a regular basis.
