JOB SUMMARY

The Occupational Therapist (OT) administers occupational therapy to patients intermittently in their place of residence. This care is provided in accordance with physician orders and a plan of care under the supervision of the Director of Clinical Services or Clinical Manager.

QUALIFICATIONS

  • Graduate of an accredited Occupational Therapy program as recognized by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA).
  • Certified by successfully completing the National Certification Examination, or eligible for the National Registration Examination of the AOTA.
  • Currently licensed in the state of practice, unless licensure does not apply.
  • Two (2) years of experience preferred.

Requirements

RESPONSIBILITIES

  • Work to improve or minimize patients' residual physical disabilities.
  • Help individuals achieve optimal and productive living within their capabilities.
  • Collaborate periodically with other home care personnel in patient care planning.
  • Provide prescribed occupational therapy as outlined in the patient's plan of care.
  • Direct and supervise personnel as required, including Certified Occupational Therapy Assistants (COTAs) and Home Health Aides.
  • Perform the initial assessment, develop and update the plan of care, establish maintenance programs, and complete reassessments every 30 days.
  • Conduct all skilled procedures as necessary.
  • Consult with physicians regarding any necessary changes in treatment.
  • Prepare progress reports for physicians on patient progress.
  • Educate patients and family members on home programs and fine motor exercises.
  • Complete an initial evaluation, including a plan of care and goals, submitted to the physician for approval. Complete recertifications every 60 days if continued treatment is required.
  • Document a progress note for each visit, perform reevaluations as needed, and write a summary upon patient discharge.
  • Present in-service programs periodically to agency staff.
  • Assist physicians in evaluating patients' levels of function.
  • Contribute to the development of each patient's plan of care and revise as necessary.
  • Maintain clinical and progress notes as required.
  • Consult with the patient's family and agency personnel regarding care.
  • Participate in in-service programs and Quality Assessment and Performance Improvement (QAPI) activities as assigned.
  • WORKING ENVIRONMENT

    Primarily works indoors in an office setting and in patient homes, with required travel to and from patient locations.

    JOB RELATIONSHIPS

    Reports to : Director of Clinical Services or Clinical Manager

    Supervises : Certified Occupational Therapy Assistant and Home Health Aide as assigned.

    RISK EXPOSURE

    High risk due to direct patient care and in-home services.

    LIFTING REQUIREMENTS

    The role requires the ability to engage in physical activities essential for patient care, including extended periods of standing and regular moderate lifting, bending, and other physical activities.
