Position Description (Full job description available upon request):
Assess individuals enrolled in waiver programs for level of care and waiver eligibility.
Essential Job Functions:
- Complete face to face assessments in the community with Consumer and caregiver/contact as appropriate to determine level of care per ODA and COA policy.
- Document in appropriate software system
- Communicate with care coordination staff as appropriate
- Communicate with Managed Care Organization per policy
- Work cooperatively and constructively with internal COA staff, and with formal and informal systems to communicate consumer needs.
- Advocate on client’s behalf particularly if there is a health and safety issue or if client is at risk for institutionalization.
- Ability to complete assessments during core business hours, early evening or weekend hours as needed to meet the needs of the community.
- Complete incident reports as indicated.
- Follow firewall requirements of confidentiality and neutrality as it relates to the MyCare Ohio Program.
Qualification Requirements:
- Ohio licensed Registered Nurse, Licensed Practical Nurse or licensed Social Worker in good standing with the credentialing board.
- At least one-year prior experience in medical social work, home health care and/or geriatrics.
- A good working knowledge and understanding of issues related to the elderly population.
- Knowledge of the individual, family, social systems and the individuals functioning in the aging process.
- Ability to collaborate and problem solve in a self-directed team environment.
- Knowledge of community resources within Southwestern Ohio.
- Good customer service skills.
- Ability to adapt to frequent change.
- Good oral and written communication skills.
- Good organizational and teamwork skills.
- A valid driver’s license and reliable transportation for travel within the COA 5-county area.
- Ability to travel within the state of Ohio for training.
- Respect for the cultural, spiritual, racial and ethnic beliefs of others.
- Excellent computer skills including Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed.
