Your duties will include:
- Providing support and education to individuals with complicated social, behavioral, and/or environmental issues through home visits and advanced care planning.
- Using clinical judgment and critical thinking skills to facilitate appropriate client physical and behavioral healthcare and coordinate psychosocial and medical wraparound services to promote effective utilization of available resources.
- Developing, assessing, and adjusting as necessary, the patient centered care plan, promoting desired outcomes along with monitoring the effectiveness of the plan and establishing measurable goals and outcomes.
- Providing coaching, information, and support to empower clients to make ongoing independent lifestyle choices
- Creating service and care plans with the most efficient means possible, using the service guidelines, individual and family input, and assessment, promoting the individual’s understanding of the service rationale.
- Meeting compliance standards established for the program including, but not limited to, timely completion of annual reassessments, adhering to documentation timeframe, timely submission of IMS reports, etc.
Qualifications:
- Ohio Licensed Social Worker (LSW), Licensed Practical Nurse (LPN), or Registered Nurse (RN) in good standing with the credentialing board.
- At least one year of experience working with people with disabilities/chronic conditions and LTSS.
- Prior case management experience and prior experience completing electronic documentation a plus.
- A good working knowledge and understanding of issues related to the elderly population.
- Knowledge of the individual, family, social and the individuals functioning in the aging process.
- A valid driver’s license and reliable transportation for travel within the five southwestern counties serviced by COA (Hamilton, Butler, Warren, Clinton and Clermont).
