II. NATURE OF WORK

This position involves the responsibility for development of care plans, screening, monitoring, counseling, charting of information, medication supervision, and medical referral/follow-up of clients. The Nurse shall be responsible to coordinate with Case Manager on an ongoing basis with matters of client health and safety.

III. POSITION RESPONSIBILITIES

  • Observe, assess and monitor participant's health needs and daily functioning levels including, but not limited to, blood pressure, pulse, temperature, and weight. Assisting clients who may be in need of physical, speech, or occupational therapy to obtain required Physicians order and arrange with subcontractor. To assess clients ability to function independently and determine areas client needs assistance with, such as activities of daily living .
  • Obtain current physician's written orders for any medication client may take during hours of attendance in the daycare center. Written orders will include any special dietary requirements and restrictions of activities. Supervise and educate clients in self-administration of medication as prescribed by the client's physician.
  • Coordinate with the Case Manager on the goals and objectives of each Adult Daycare client. Detail areas of special client interest and what is hoped to be gained. Refer to other community resources with follow-up of suspected physical, mental or social problems requiring definitive resolution including, but not limited to, cases of abuse or neglect. The nurse and the clients case manager should keep open lines of communication and should assist each other in providing the appropriate and highest quality of care to the client. Encourage group interaction and socialization.
  • Provide education to clients and caregivers in matters relating to safety in the home, proper personal hygiene, health and prevention of illness.
  • Responsible for updating procedure on emergency first aid. Provide training to staff members in cases of medical emergency.
  • Monitor the daily attendance log with phone call to each absent client. Notification to Program Administrator, RN and the Executive Director of any client condition requiring a two day or more absence. If appropriate, assist in computerizing clients records.
  • Assist those persons responsible for meeting the regulations for the Department of Education Adult Food Program, which includes but is not limited to developing nutritious snacks that meet the requirements of the Program. Maintain accurate records including client applications for the food program, roster, and attendance records.
  • Assist staff as needed in planning and implementing recreational programs and social activities including exercise, outings that are appropriate to the client base, etc.
  • Supervise clients at lunch period. Assure that all staff are trained in proper procedures to prevent or assist client who may have the potential of choking. Ensure that the clients given a choice on food items when possible.
  • Supervise the giving of personal care to clients in the day care program who may be eligible to receive personal care during the hours they attend Adult Day Care. Personal care to include but not limited to shower, haircare, nailcare, assistance as needed.
  • Report unusual behavior or changes in clients condition to appropriate persons including if necessary clients physician, caregiver, Case Manager or ADC Director, RN.
  • Ensure that any medications stored in the Adult Day Care Center are in a properly labeled container with the clients name, medication name, dosage frequency and method of administration. Medications will be kept in a locked cabinet or draw.
  • Maintain current drivers license and reliable transportation for in-home client visits, should the need arise.
  • Advise or suggest to Council on Aging of St. Lucie, Inc. any medical equipment that may be necessary to provide care to clients such as portable oxygen, first aid kit, recitation equipment, etc.
  • Prepare and schedule in-service and pre-service training as required.
  • In the event the President of the United States or the Governor of the State of Florida declares a disaster or a state of emergency, the Adult Day Care Nurse shall perform his/her duties under the direction of the Executive Director as prescribed by the COASL Disaster Preparedness Plan in order to implement emergency relief measures and/or activities. All actions under this plan shall be for the purpose of ensuring the health, safety and welfare of the elderly in the disaster area.
  • Display initiative in developing new procedures and presenting new ideas to supervisors that may improve the Day Care Program. Maintain contact with nurses in other day care programs for the sharing of ideas.
  • Ensure that the client is treated as an adult not a child.
  • Maintain a courteous and professional demeanor at all times with supervisors, co-workers, clients, clients family and the general public. Ensure that the Client is treated with respect, dignity and right to privacy.
  • Other duties as required by the ADC Director, RN.
