Supervisor: Director of Senior Nutrition and Health Services
Salary Range: $40 - $46 per hour
Program Description: The Clinical Supervisor provides professional guidance, supervision, and training to both staff and volunteers in the assessment and care planning for senior clients in the Peer Support and PEARLS mental health programs. The Senior Peer Support program matches trained community volunteers with local seniors in need of focused support while they cope with mental health challenges. PEARLS supervision assists PEARLS Coaches with guiding clients who are depressed or socially isolated. The clinical supervisor will have support from a volunteer coordinator in securing volunteers for Peer Support. The Clinical Supervisor will have a role in outreach regarding mental health services available at the Council on Aging and in the community at large.
Typical Physical Demands: Sitting; standing; climbing steps; getting in and out of the vehicle; walking; ability to manipulate tools, controls, and keyboards; ability to write; exposure to cigarette smoke, pets, and extreme home conditions.
Typical Work Conditions: Work is performed both in the COA office and in the field, at client homes or other community locations.
Responsibilities:
• Supervise, and support up to 15 active volunteer Senior Peer Support (SPS) counselors with weekly group meetings and if needed individual sessions.
• Once yearly hold an SPS training sessions to recruit more volunteers into the program.
• Collaborate with other Council on Aging programs and partners in the community to generate appropriate client referrals.
• Screen potential SPS clients to assure they are appropriate for the program.
• Manage the SPS program in compliance with contract requirements and Council on Aging policies and procedures.
• Attend staff meetings and trainings as required by supervisor. This includes meetings held outside COA which are part of contract compliance. Meet with supervisor for weekly check-ins.
• Establish and maintain client files as per contract and established professional guidelines.
• Assist Director in completing quarterly and monthly reports.
• Work as a member of the Council on Aging team to identify appropriate additional support for clients.
• Be available for consultation with volunteers and/or clients regarding urgent program-related issues.
• Demonstrate knowledge of all COA programs and services and other community resources to support older adults. This includes completing PEARLS training and observing all other COA programs.
• Perform Intake & Assessment, for SPS clients.
• Develop a Care Plan for SPS clients, which includes specific, quantifiable goals that address each identified need, and strategies for each goal.
• Perform ongoing in-person meetings, telephone check-ins, and reassessments required for each program, monitoring and updating the Care Plan as needed.
• Track client progress, recognize successes, and identify barriers to ensure client satisfaction.
• Record case information, complete accurately all necessary forms and databases, and produce KPIs and reports.
• Other duties as assigned by the Director.
