Title: Elder Advocate Coordinator
Supervisor: Lead Elder Advocate at Family Justice Center
HOURS: Monday-Friday 37.5 hours a week (Temporary grant-based position for 6 months with potential for full-time if renewed)
SALARY RANGE: $27.00 - $28.00 per hour
ROLE: Provide trauma-informed and client-centered care coordination to adults age 60 in Sonoma County who are victims of domestic violence, physical, emotional or sexual abuse, and/or financial exploitation as directed by the Lead Elder Advocate which includes addressing the needs of victims of abuse facing housing insecurities when applicable. The primary focus of this role is to empower clients to change their circumstances, stabilize their lives, and navigate the criminal justice system while receiving the continued support and protection necessary to achieve these goals. The Elder Advocate Coordinator will provide assessment, information, and referrals to appropriate agencies and other resources including support groups, advocacy services, and prevention tools.
RESPONSIBILITIES:
- Receive and respond to referrals from Community Based Organizations such as Legal Aid of Sonoma County, the Family Justice Center Sonoma County (FJCSC), Adult Protective Services (APS), Ombudsman, Verity and other appropriate community organizations in a timely and professional manner.
- Conduct comprehensive assessments for services and to identify the clients’ areas of concerns. Develop individualized care plans with clients based on the clients’ condition and situation. Address areas of concerns, obtain services according to the individualized care plan and coordination of existing services.
- Assist the senior victim with crisis and/or family intervention, safety planning, dealing with police and/or the justice system, accessing support systems including in-home help, changing living situations, facilitate other services needed to stabilize the seniors’ lives, and performing additional tasks as appropriate.
- Maintain accurate, up-to-date client records, including but not limited to; intake and assessment forms, progress notes, care plan, reports, and other administrative duties as assigned.
- Strive to educate and empower clients, and be a voice on their behalf with law enforcement, legal and court systems and other service providers.
- Adhere to Sonoma County Human Services Department, Adult and Aging Services, Council on Aging’s and Family Justice Center’s policies, procedures, standards, and values including compassion, respect, inclusiveness, service excellence, confidentiality, and teamwork.
- Shadow Lead Elder Advocate to learn and document through "Standard Operating Procedures", all standard protocols for Elder Advocates dealing with Law Enforcement, District Attorney's Office, Adult Protective Services, Family Justice Center, FJC onsite partners, and abuse victims.
- Attend Multidisciplinary Team (MDT), Adult Protective Services, Council on Aging, Family Justice Center, and other relevant meetings and workshops as assigned to expand areas of expertise and maintain working relationships.
- Develop and implement an elder abuse awareness outreach plan and provide community resources and education as needed.
- Research existing and potentially new sources for elder advocacy funding; prepare and/or assist agencies in preparation of grant applications or proposals.
