POSITION DESCRIPTION
Care Coordinator
POSITION PURPOSE & GENERAL DESCRIPTION:
To assist clients to improve their overall health and to improve client’s satisfaction of their health. To provide assistance with members behavioral health, physical health and environmental factors through a variety of modalities. To assess the consumer’s strengths, assess mental health, physical health and social needs then develop and implement an individualized service plan designed to improve the client’s overall health and quality of life.
ESSENTIAL FUNCTIONS:
The following are fundamental duties that an individual who holds the position must perform. This is not an exhaustive list. Other duties, in addition these essential job functions, will be assigned at the discretion of the Program Director.
Care Coordinator:
- Determine the member’s strengths, needs, mental status and physical health needs using state approved assessments.
- Coordinate with the member to identify goals and write an approved plan for maintenance and recovery.
- Conduct monthly contact with each member via face-to-face visit or by phone. Assist to appointments when necessary.
- Provide members with continued education and reinforcement of ongoing whole health needs.
- Assist in referring or coordinating community resources identified for the consumer.
- Coordinate communication with members including appointment scheduling, arrangement for transportation and follow up calls with members.
- Follow-up on missed appointments by consumers.
- Cooperate with payor sources in providing needed information. Payor sources include managed care organizations, Iowa Medicaid Enterprise, and Southeast Iowa Link.
- Review patient profiles with IHH Health Team to assess needs of consumers. Make internal referrals when necessary to Nursing, Peer Support, Behavioral Interventionist, and Therapy.
- Work independently to schedule new referrals and home visits while managing time effectively.
- Document the service that has been provided and complete required/requested external and internal reports and other paperwork in a timely manner
- Complete all necessary paperwork for insurance and billing. Adhere to all deadlines set forth by Counseling Associates and the payor source.
- Attend all team meetings, coachings, and care coordination activities and case discussions for members.
- Assist in data collection and reporting, as directed by the Program Supervisor.
- Participate in on-call rotation.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE:
1. Bachelor’s degree in social work or related field.
2. Experience in providing direct client services essential with prior experience in a community outreach and coordination of services preferred.
3. Basic computer skills.
CERTIFICATES, LICENSE AND REGISTRATIONS:
- Possess a valid driver’s license, proof of auto liability insurance and a good driving record.
- Possess a valid social work license in the State of Iowa.
SKILLS, KNOWLEDGE & ABILITIES:
1. Ability to professionally relate to a wide variety of consumers, as well as a desire to serve as a consumer advocate.
2. Ability to collaborate and function as a team member with multi-disciplinary colleagues.
3. Ability to manage multiple tasks at one time in an ever-changing, dynamic environment and utilize good organizational skills.
4. Practice good verbal and written communication skills with ability to complete case notes and complete required assignments in a timely manner.

RISK CLASSIFICATION EXEMPT:
This position requires tasks that involve no greater exposure to blood, fluids or tissues than would be encountered by a visitor. The normal work routine involves no exposure to blood, body fluids or tissues and the worker can decline to perform tasks which involve a perceived risk without retribution.
