We have an opening at our Sand Springs location. The role of a Family Support Provider (FSP) II / Wellness Coach requires a High School diploma or equivalent. A candidate raised or lived with a child who has emotional problems is preferred with a minimum of 10 years' experience as a Family Support Provider (FSP). Experience in seeking and accessing state and community resources and a willingness to help families access resources.

The starting salary hourly rate for a Family Support Provider is $20.00

Counseling & Recovery Services of Oklahoma provides mental health and substance abuse treatment, physical health care management and wellness services for over 5,000 children and adults annually. We are Oklahoma's only community mental health center certified in the Sanctuary model of care. Special Knowledge, Skills and Abilities :

  • Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organize and present facts effectively.
  • Must possess good working knowledge of community social services resources, strong-emotionally intelligent advocacy skills, basic crisis management skills, behavioral health resources, and the ability to provide trauma-informed care and a commitment to the Sanctuary Model components.
  • Ability to communicate clearly and in an emotionally intelligent manner with clear, polite, and respectful communication with staff, client's and families, administration, public school personnel, and the community-verbally, in person, through written correspondence, and on the telephone.
  • Exhibit ability to effectively organize and plan field-work schedule to best service and maximize the needs of clients, ie. arranging home based meetings, agency meetings, care coordinator meetings.
  • Ability to work well under pressure meeting deadlines with efficiency and consistency while maintaining appropriate self-care with a balance between good physical and mental health and the job requirements.
  • Creating a family plan and course of action based on individual needs of a family.
  • Using personal judgment, identify available options for agencies and organizations as appropriate to meet a particular family's needs.
  • Evaluating situations using discretion and personal judgment to adopt an effective course of action, to delegate responsibilities, organizes facts and presents them effectively.
  • Advocating for the family in the agency setting, and with other agencies and organizations identified.
  • Using discretion and independent judgment to interpret and implement policies regarding family plans and advocacy.
  • ESSENTIAL DUTIES AND RESPONSIBILITES :

  • The Family Support Provider (FSP) is to provide targeted support, advocacy, education, behavioral support and mediation / collaboration type services within a system of care that is child-centered and trauma-sensitive with the needs of the child and family dictating the types and mix of services provided, to assist in preventing an out-of-home placement.
  • Responsible for providing direct / indirect advocacy, behavioral support, education, case management and / or rehab services to maintain agency productivity requirement of 25 hours billable service per week.
  • Complete initial care plans, strengths and culture discoveries and intakes as needed, update and modify care plans as needed. Create a family plan and course of action based on individual needs of a family.
  • The FSP II will maintain close communication with the Project Director, Care Coordinator, Therapists, Physicians, etc.. and other clinical and administrative staffs in order to assure the highest quality of services are delivered to the client and his or her family.
  • To assist in preventing an out-of-home (or school) placement and / or in facilitating discharge from an inpatient or residential treatment facility.
  • To provide targeted support and advocacy services (behavior management, daily living skills, social skills); linkage with community supports; referral to local needed services; advocacy; service coordination; and follow up on behalf of client and their family who are experiencing emotional and / or behavioral challenges that are significantly interfering with their success at home, school, work or in the community.
  • Monitor and evaluate services provided to the families. Contact families as needed to monitor progress and inquire about new needs; Complete flex-fund requests when necessary. Provide transportation for clients and families to appointments, trainings, and meetings when a bus or cab is not appropriate.
  • Responsible for teaching about and implementing regular community meetings and safety plans with client(s) as part of commitment to the Sanctuary Model of client care.
  • Be a member of the family team to assist in the strengths assessment and care plan development. Recognize the individual child client and his / her family has different barriers to social, economic, and employment opportunities with varied levels of need to overcome these barriers. (The goals for each juvenile client as identified in the family team will be tailored to reflect a juvenile client's particular needs in order to realize her / her full potential).
  • Coordinating services between agencies and developing cooperative working relationships with agencies serving children and families.
  • Have knowledge of psychotropic medications and their actions.
  • Other Duties : Perform other duties as assigned by Supervisor.

    Physical Requirements : Must be able to move from location to location, bending, stooping, lifting 25 pounds, communicate clearly, prepare written communications, present information to other employees, process complex information within established procedures / programs, enter data to various programs. Must be able to climb stairs and access non-handicapped equipped residences / facilities.
