What are the responsibilities and job description for the Service Coordinator position at Counseling Service of Addison County, Inc?
This case management position works to manage and supervise all aspects of service for individuals with intellectual disabilities, autism spectrum disorder, Traumatic Brain Injury and/or elderly. Work within a multidisciplinary team to develop, plan, and implement case management across community and residential settings. The Service Coordinator is responsible for the supervision of Direct Service Professionals, monitoring and maintaining clients' needs related to employment, housing, health and wellbeing, adhering to strict documentation standards and fostering positive relationships with community partners. A successful candidate would have the ability to problem solve independently, collaborate within a team, successfully time manage a fast-paced work environment with a positive outlook. Qualifications:
A Bachelor's degree in a relevant field and one year of direct experience or a high school diploma or GED and have a minimum of six years of relevant experience preferably with some experience working directly with individuals with developmental disabilities but not required.
