WOUND CARE NURSE JOB DESCRIPTION1. All new wounds will have a documented initial measurement and assessment performed by the nurse discovering the wound or when it is reported to the nurse by CNA/CMA. If the wound care nurse is in the facility, she must be notified immediately for assessment or assess any new wounds on his or her next scheduled shift.a. The nurse will then obtain initial wound care orders from MD and notify resident family members. All notifications must be documented in the progress notes.b. The nurse will then notify the wound care nurse of any new skin issues and initiate order to refer to Vohra wound physician.2. The wound care nurse will follow up by assessing the wound immediately and then weekly and obtaining new measurements with the appropriate documentation, including updating the TAR with any new orders. (Vohra residents weekly on Mondays.)3. The wound care nurse will implement interventions for the resident with wounds, such as: Tx orders, dietary supplements, labs, positioning, staff education with documentation of such intervention.4. The wound care nurse will notify MDS Coordinator of all new wounds and interventions set in place for the care plan5. If the wound is not improving or resolved in 2 weeks the MD wound care physician must be notified for change in plan of care. The family/POA will be notified of wound progress6. Floor nurses will perform daily wound care that will be monitored by the wound nurse.7. The wound care nurse will make weekly rounds with the wound care physician on each resident seen by Vohra. All new measurements and orders will be documented and updated on the ulcer/wound report or wound/non-pressure report. A weekly wound report and TAR and notify family each week on progress.8. The wound care nurse will perform weekly head to toe skin check on all residents for possible skin breakdown and obtain any new orders and measurements as needed with appropriate documentation as well as notifying family.9. For any new in house acquired or worsening wounds there will be a wound investigation to be completed and turned in to the DON.10. All admits and readmits will have a second a “second look” at the skin assessment and skin assessment documentation and have a skin assessment performed by the wound care nurse if not available at time of admission on his/her next scheduled shift.11. Wound care nurse will be responsible for:a. Inservice with staff on wound prevention quarterly with new hires and PRN.b. Skin assessments and documentationc. Administering skills checkoffs to ensure the competency of all nurses on the floor.12. Infection Control Nurse, Infection Control Educator, and Control Resource Specialist.13. Educating staff/residents on infection prevention, identifying sources of infection, interpreting surveillance data to identify infection and risk of infections14. Respond to outbreaks and manage incidents of infections.15. Communicate with Public Health Organization in relation to infection control.16. Ensure facility equipment is clean, disinfected, or sterilized according to guidelines.17. Ensuring isolation procedures are met.18. Responsible for assisting with obtaining immunization consents, administering and keeping accurate logs/documentation of residents.19. Responsible for administering staff vaccines and keeping accurate logs/documentation.20. Inservicing staff on infection control upon hire and quarterly as well as PRN.21. Skills competencies regarding infections control, peri care, handwashing, etc.
Pay: $28.00 - $32.00 per hour
