Performs intermediate skilled human service work by providing person-centered treatment, recovery, and/or support services to assigned clients in behavioral health and long-term support programs in the community. These programs consist of the Comprehensive Community Support Program (CCS), Coordinated Services Team (CST), Community Support Program (CSP), and Children’s Long-Term Support Medicaid Waiver Program (CLTS).  Is responsible for conducting assessments, developing and implementing person-centered service plans, coordinating service delivery, providing ongoing case management, assessing crisis needs, and providing client advocacy for assigned clients. Will participate in the after-hours mobile and on-call crisis system. The work is performed under the direction of the Coordinated Services Manager and/or Supervisor. 

  1. Utilizes a trauma-informed approach to provide person-centered treatment, recovery, and support services to clients in behavioral health and long-term support programs.

  2. Performs assessments for person-centered plans.

  3. Coordinates delivery of services described in the client's recovery plan.

  4. Organizes monthly, semi-annual, and annual team meetings.

  5. Maintains client records in accordance with regulatory guidelines.

  6. Provides accurate and timely case management documentation.

  7. Participates in after-hours mobile and on-call crisis system.

  8. Serves as point of contact for the Health and Human Services Department relative to assigned external agencies, programs, or services and represents the department at professional meetings and community events as required.

  9. Consults and receives supervision from other professional staff within the agency.

Bachelor's degree in vocational rehabilitation, psychology, social work, or related field and moderate experience working with specified target groups (e.g., children with disabilities, individuals with behavioral health needs, etc.), or equivalent combination of education and experience. Certified Social Worker preferred. Certification as long-term care and children’s functional screener.  Must pass a Wisconsin Department of Justice and Department of Health Family Services background check.  Valid Wisconsin driver's license.

OTHER KNOWLEDGE, SKILLS & ABILITIES

Thorough knowledge of County and department policies and procedures to include emergency and inclement weather policies; ability to prioritize and manage multiple, concurrent projects; attention to detail and ability to effectively organize time and anticipate, plan and successfully respond to changing circumstances; thorough knowledge of conducting assessments for person-centered plans; thorough knowledge of case management documentation; ability to maintain client records in accordance with existing policies and procedures; ability to make arithmetic computations using whole numbers, fractions and decimals; ability to operate and use standard office equipment; knowledge and ability to use standard office and position-specific software; ability to effectively communicate both verbally and in writing; ability to establish and maintain effective working relationships with co-workers, supervisors, other departments and the general public.

PHYSICAL DEMANDS 

This work requires the occasional exertion of up to 25 pounds of force; work regularly requires speaking or hearing and using hands to finger, handle or feel, frequently requires sitting and occasionally requires standing, walking, climbing or balancing and lifting; work has standard vision requirements; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly or quickly; hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound; work requires preparing and analyzing written or computer data, operating machines, operating motor vehicles or equipment and observing general surroundings and activities; work occasionally requires exposure to outdoor weather conditions and exposure to bloodborne pathogens and may be required to wear specialized personal protective equipment; work is generally in a moderately noisy location (e.g. business office, light traffic).
