BASIC FUNCTIONS:

• Under the general supervision of the unit Social Services Supervisor, provide mental health services to children and families within the school, home and community settings; mental health services may be early intervention and/or ongoing intensive case management. Provide services to children who are eligible for special education services or are at risk to receive these services.
• The Family School Support Worker (FSSW) is a licensed social worker employed by the county but primarily located in an assigned school setting. The FSSW works cooperatively with schools, the county social service agency, and community resource agencies.

ESSENTIAL FUNCTIONS:
Case Management

• Respond to referrals from various sources regarding children who are at risk and/or have mental health/behavior needs.
• Assess child and family needs in order to assist the family in obtaining mental health, social, educational, financial, health, recreation, advocacy, transportation, and legal services. Provide resource information and make referrals.
• Determine eligibility for case management services. Determine if child meets Indian Child Welfare Act (ICWA) criteria. Document eligibility in SSIS.
• Create and maintain a social service case file, including the completion of all county, children’s mental health and/or child welfare paperwork according to guidelines.
• Provide children’s mental health case management services according to MN Statute and/or MN Rule 79.
  • Assist the family in obtaining a comprehensive diagnostic assessment completed by a mental health professional.
  • Complete a functional assessment on the child within 30 days of determining eligibility for services. Update this assessment every six (6) months.
  • Develop an Individual Family Community Support Plan (IFCSP) to address the needs of the child based upon the child’s diagnostic and functional assessments. Complete within 30 days of determining eligibility and update this plan every 6 months. For clients age 14 years and older, complete an Individual Living Services (ILS) plan in addition to IFCSP.
  • Monitor the child’s progress toward IFCSP goals, continue to assess the child’s needs and appropriateness of services and make recommendations for additional services. Make a minimum of monthly, face to face contact with child and/or child's parent(s). Make regular contact with collateral contacts.
  • Apply various treatment approaches with the individual or family based upon the situation and needs of the client.
• Assess need of families for Parent Support and Outreach case management services.
• Provide and facilitate Family Support Grant service to qualifying families.
• Provide crisis intervention services including referrals to meet emergency needs for housing, food, clothing or financial assistance.
• Assist client, directly or by making referrals, in coping with mental health and practical issues encountered due to mental health diagnosis.
• Develop, implement, coordinate and maintain safety plans for client safety concerns.
• Submit and receive approval for expenses with fiscal responsibility.
• Receive individual and group clinical supervision of mental health practice a minimum of 1 hour per month.
• Adhere to ICWA requirements and procedures as related to child welfare and children’s mental health service delivery.
• Complete and maintain 100% time recording of all case management.

Out of Home Placement

• Coordinate all out of home placement activities for children and adolescents according to MN Statute 260D.
• Contact appropriate tribe and coordinate pre-placement court hearing for ICWA-eligible children.
• Collaborate with mental health service and insurance providers to obtain prior authorization of placement.
• Schedule and facilitate pre-placement meeting with family to review Title-4E needs, treatment team members and responsibilities of members.
• Meet monthly with all children in placement.
• Schedule administrative and court reviews at 6 and 12 months. Comply with court requirements for out of home placements. Complete court reports in a timely manner and submit to PHHS Paralegal for e-filing. Attend court hearings. Maintain contact with the County Attorney’s office.
• Comply with state and federal statutes pertaining to out of home placements in order to meet requirements of Child and Family Service Review Audits.
• Assume case management and placement coordination for Human Development Center (HDC) and Fond du Lac (FDL) case managers when a child is placed out of the home. Comply with timelines when placing an ICWA-eligible child.
• Facilitate discharge planning with treatment staff, client and family, and community support service providers to insure proper services are obtained prior to discharge according to treatment recommendations and client/family needs.

Mental Health Practitioner Services

• Triage with school staff regarding children who are identified as having needs (emotional/behavioral issues, school issues, peer issues, family/home issues, attendance issues, etc.). Assist in planning and implementation of services.
• Participate in special education and pre-referral teams within the school (IEP teams, Children Study Team, Student Assistance Team, etc.).
• Provide an array of services to school staff, children, and families which may include consultation, assessment, individual meetings, parent/family meetings and service referral/coordination.
• Teach and prompt appropriate parenting skills and follow through.
• Participate in the preschool screening process. Complete the ASQ-SE as requested.
• Participate on school crisis teams, as requested, to provide grief and crisis support to students and staff related to a school crisis or death.

Children’s Mental Health Office Worker of the Day

• Respond to calls, faxes, or emails requesting children’s mental health case management services.
• Complete required forms, SSIS documentation and notifications.
• Provide information, consult and refer professional services regarding children’s mental health issues to the general public, families and the community.
• Coordinate and insure office coverage to respond to CMH referrals.

MINIMUM QUALIFICATIONS:
Education and Experience:

• Bachelor’s Degree in Social Work or Human Services related field required.
• One (1) year related professional work experience in mental health, counseling, social work, child protection or corrections.

Training Required:

• Must complete Department of Human Services (DHS) facilitated Children’s Mental Health and SSIS trainings within one (1) year from date of hire. Must complete bi-annual continuing education requirements for social work license.

Certificates/Licenses Required:

• Licensed Social Worker
• Valid driver’s license and proof of insurance.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

• Knowledge of mental health issues relating to children and adults, child development and family dynamics.
• Knowledge of mental health diagnosis for children according to the Diagnostic and Statistical Manual (DSM).
• Knowledge of MN Statute, MN Rule 79 and the National Association of Social Work Code of Ethics.
• Knowledge of and ability to implement relevant laws, regulations and policies regarding social service provision, reporting requirements, data practices and client rights.
• Knowledge of parenting resources, community resources and the legal system.
• Knowledge about the special education process.
• Skill in interviewing techniques, assessment, case planning, and case management.
• Skill in interventions with individuals and families.
• Ability to communicate effectively and professionally, both verbally and in writing.
• Ability to develop positive working relationships with diverse people and personalities
• Ability to make independent judgments with minimal consultation.
• Ability to tolerate high levels of stress.
• Ability to lift and carry up to 20 pounds.
