Summary
The Program Specialist coordinates, implements, analyzes, evaluates, monitors and coordinates a number of ancillary programs and services within the department. These programs, services and duties are specialized as per the client goals specific to the service unit.
This job requires the following documents to be obtained, reviewed as acceptable by the supervisor and re-obtained every five years : PA Child Abuse, PA Criminal History and FBI Criminal History.
A person in this position is expected to demonstrate the System of Care Practice Model which maximizes all resources around an individual or family. This system of care includes child and adult serving agencies and community partners that join with an individual or family to meet their goals.
Essential Duties
Essential Duties, Tasks and Accountabilities :
- Plan, develop and implement program
- Monitor, analyze and evaluate results of program effectiveness
- Advice on program problems and objectives
- Recommend programmatic change related to program goals and objectives
- Act as liaison to service providers
- Provide information / training / support to department staff regarding access to program services
- Ability to collect, analyze and interpret data
- Provide program liaison function to identified contracted provider agencies or programs to include
participation in contract development meetings, referral and discharge process oversight, problem-solving issues with providers and ensuring communication between department and provider staff, monthly invoice
reviews, schedule and or participate in regular meetings with provider agencies, arranging monitoring
visits and writing related reports, enter monitoring information in database system.
