OVERALL OBJECTIVE OF JOB
Duties include provide planning, coordinating, and evaluation of services for the Child and Adolescent Service System Program (CASSP) target population (i.e. children and adolescents with serious mental health problems) and to those at risk of developing serious mental health problems. System of Care coordinator-serve as the county liaison for the System of Care (SOC) project for the duration of the grant. Child and Community Liaison- Act as a liaison between the Clarion County Mental Health Dept. and area child service agencies. .
ESSENTIAL FUNCTIONS OF JOB
- CASSP Coordinator
- Serves as a liaison with the MH/DD, Children and Youth Services, Riverview Intermediate Unit, public and private schools, Juvenile Probation, Drug and Alcohol treatment facilities, mental health treatment facilities and insurance companies to coordinate service provision to the CASSP target population.
- Obtain information from client, family members and other service providers: evaluates and relates that information to present behavior and emotional problems of the child/adolescent, works with the family and participating CASSP providers to arrange timely and appropriate service.
- Schedules and conducts CASSP meetings
- Prepares initial Residential Treatment Facility (RTF) packets and supporting documentation for authorization.
- Monitors the care of children/adolescent in residential placement, participates in reauthorization and monthly update meetings and co-facilitates the discharge planning for those placements.
- Facilitates the Children’s Service and Support Team meetings for cases involving the CASSP target population and prepares a written document identifying options to address unmet needs.
- Provides consultation to schools and local agencies involved with children and adolescents from the CASSP target population.
- Assist families, children/adolescents, schools and provider agencies with the IBHS service, process and procedures.
- Submits all pre-certifications for Family Based Mental Health Services. Screen requests for appropriateness of respite services and approves or denies the referral.
- Participates in the Mental Health providers meetings.
- Maintains records and complete required reports.
- System of Care Coordinator
- Serve as the county liaison for the System of Care (SOC) for the duration of the grant.
- Represents Clarion County for SOC matters.
- Prepare and submit a grant budget for approval by the Mental Health Administrator or Deputy Mental Health Administrator.
- Create and maintain a County Leadership Team (CLT) in accordance with Tier 1 County requirements.
- Create, support, and promote community programs that enhance, improve, establish or invest in mental health services for children/adolescents.
- Conduct or provide training to the community that promotes Trauma Informed Care principles.
- Communicate with BHARP and its system partners in accordance with SOC requirements.
- Child and Community Liaison
- Act as a liaison between the Clarion County Mental Health Department and the area child service agencies to include but not limited to: schools, Juvenile Probation, service providers, daycares, placement facilities, CYS, libraries, recreation facilities and churches.
- Attend and support local youth involved outreach programs.
- Communicate with local agencies on gap, needs or trends with local youth and report findings to the Mental Health Administrator/Deputy Mental Health Administrator.
- Advocate for programming, services or funding for the youth and adolescents of Clarion County.
- Collaborate with organizations that serve Autism, Drug and Alcohol, Suicide Awareness, and Independent Living.
- Continue assisting, monitoring or communicating with SOC projects and programs after completion of the grant.
OTHER DUTIES OF THE JOB
- Attend staffing, training, and other meetings as required.
- Proficient with computer programs.
- Performs other job-related work as required.
SUPERVISION GIVEN
Reports directly to the Mental Health Deputy Administrator and receives occasional instructions and supervision regarding daily work duties.
WORKING CONDITIONS
- Works indoors with adequate lighting, temperature, and ventilation.
- Works with average indoor exposure to noise and stress, but subject to frequent disruptions.
- Works with average indoor exposure to dust/dirt, noise, stress, and disruptions.
- Travels periodically to other work sites as needed.
PHYSICAL AND MENTAL CONDITIONS
- Must possess ability to record, convey and present information, explain procedures and follow instructions.
- Must possess hearing, seeing, speaking and reasoning senses necessary to carry out job duties.
- Must be able to sit for long periods during the workday, with frequent standing, walking and occasional bending, twisting, stooping, reaching and grasping as necessary to carry out essential job duties.
- Dexterity requirements range from simple to coordinated movements of fingers/hands; to simple movements of feet/legs/torso as necessary to carry out job duties.
- Light work, with occasional lifting/carrying of objects with weights of 20-30 pounds.
- Must be able to cope with the physical and mental stress of the position.
- Must be able to pay close attention to details and concentrate on work.
QUALIFICATIONS
EDUCATION/EXPERIENCE
Bachelor’s Degree in a related social service field. Two years of experience working in the field of mental health.
Necessary Special Requirement: Must possess a Pennsylvania driver’s license.
CLEARANCES:
- Must work in conjunction with employer to apply for Act 34 PA State Police Criminal History, Act 151 PA Child Abuse and FBI clearances prior to employment. All clearances must reflect acceptable results.
- Must not appear on Preclusion Lists as defined by “Pennsylvania’s Medical Assistance Bulletin 99-11-05 Provider Screening of Employees and Contractors for Exclusion from Participating in Federal Healthcare Programs.”
