Goodhue County Health & Human Services is seeking qualified applicants for a Care Coordinator.
Provide case management, care coordination and comprehensive assessments to Goodhue County residents.
Examples of Work:
- Complete MNChoices or South Country Health Alliance/Blue Plus comprehensive assessments in home and community settings.
- Assess and evaluate client’s health and living situations to determine level of care and services needed.
- Develop person-centered care plans to address service needs in the community
- Collaborate with community providers to meet client needs
- Provides input in program planning, the development of agency goals and the evaluation of the agency’s programs & policies.
- Manage individual budgets for approved services
- Maintains case file documentation.
- Work as part of the Home and Community based services and Health plan team.

Minimum Qualifications
Bachelor’s degree in nursing and certification as a public health nurse with current licensure as a registered nurse in Minnesota or current licensure as a registered nurse OR
Bachelor’s degree in social work, psychology, sociology or other closely related human services field.
Must possess a valid driver’s license. Ability to complete and pass MNCHOICES certification upon hire.
Compensation/Benefits
County employees receive competitive pay and a generous benefit offering. The Care Coordinator is compensated at salary grade 112 ($71,718.40 - $111,196.80) and starting annual compensation is $71,718.40.
