Under the supervision of the Division Director, develops, organizes, promotes, and implements health risk reduction and wellness programs. Activities include screening, prevention, and education in community, worksite, and school settings. May be assigned to coordinate programs and projects on a regional basis including: planning and coordinating conferences and other activities, serving as technical advisor, maintaining documentation, and preparing reports.

1. Coordinates services with other divisions of the department as well as outside agencies in identifying community needs and planning, developing, promoting, providing, and evaluating information or programming. Meets with community and business leaders to inform and garner support for health issues and programs.

2. Coordinates the distribution of mini grants to service providers with assigned areas. Evaluates the performance of service providers.

3. Develops, implements, and monitors contractual agreements for needs assessment and facilitation of strategic planning.

4. Meet with clients to provide harm reduction supplies and services as well as linkage to other community supports.

5. Provides technical assistance to program staff in other agencies throughout the region and coordinates programs and projects with other agencies. Includes scheduling and coordinating meetings and conferences, recording and reporting on outcomes, maintaining documentation, and preparing reports for the State.

6. Coordinates preventative outreach programs, includes coordinating programs in schools, developing a health education plan for schools and Headstart sites, and establishing screening, awareness, and educational programs.

7. Collects and analyzes data for program planning and evaluation. Maintains records for assigned program areas and prepares activity and statistical reports regarding services.

8. Enforces the County Tobacco Ordinance, includes responding to complaints regarding youth access to tobacco and investigating and citing violators as necessary. Responds to complaints regarding the Clean Indoor Air Act and takes an educational approach to resolution. Responsible for the management of tobacco license renewals.

9. Assists in researching federal and state funding programs to determine conditions and criteria for eligibility of health department participation. Prepares applications for grants and other funding alternatives. Assists in developing program budgets.

10. Coordinates volunteers and programming that support public policies and health issues and assists in the development of program plans and grants.

11. Serves on various committees with representatives of various other groups within the community that are related to health education issues.

12. Develops and conducts health education/promotional presentations to community groups. Presents programs on health risk reduction and wellness in community, worksite, and school settings.

13. Researches and prepares current materials, literature, statistics, and other resources for program implementation. Promotes collaboration among health providers in the region.

14. Drafts or compiles brochures, reports, manuals, newsletters, pamphlets, posters, and various training materials promoting public health issues and health department activities. Prepares displays for specific programs. Purchases media materials.

15. Organizes and conducts orientations, conferences, workshops, seminars, and other health related educational programs.

16. Meets with families referred by the Michigan Department of Health and Human Services (MDHHS) to focus on issues such as parenting, hygiene, nutrition, housing, education, and money management. Assesses progress and prepares reports.

17. Assists with mass vaccination clinics, including drive-thru.

18. May complete HIV/AIDS or Hepatitis C counseling and testing with clients. Assesses and counsels high-risk individuals regarding Hepatitis C and HIV antibody testing and risk reduction behavior. Draws blood samples for Hepatitis C and HIV antibody testing.

19. May perform duties such as purchasing and ordering supplies, packaging supplies for distribution, and preparing mailings.

20. Function in a response capacity to public health emergencies consistent with knowledge, skills, experience and professional licensure/certification. Acquire knowledge to be competent in appropriate response roles.

Education: Bachelor’s Degree in Health Education, Public Health or a related health field. May require specific coursework unique to specific assignment.

Experience: Two years in a health promotion/education capacity. May require specific experience and skills unique to the specific assignment.

Other Requirements:

· Valid MI Driver’s License

· May require MDHHS Certification in HIV/AIDS counseling and testing.

· May require training in Bloodborne Pathogens, Biohazardous Waste, Hazard Communication.

· May require certification in CPR.

· May require training in ICS.

Physical Requirements:

· Ability to walk over uneven terrain and climb stairs to gain access to worksite locations.

· Ability to enter and retrieve information from the computer.

· Ability to lift and carry boxes, display cases, audiovisual equipment, and other materials weighing up to 40 lbs. Places material in, and retrieve from, a vehicle.

· Ability to bend and stoop to access office files and place/retrieve items from storage units.

· Ability to perform physical requirements which may vary with the assignment.

Working Conditions:

· Works in office conditions and at worksites throughout the County.

· Travels throughout the County in all types of weather conditions.

· May be required to travel throughout the U.P. and downstate for trainings / conferences.

· May be exposed to communicable diseases, blood and other bodily fluids, as well as other toxic chemicals.
